Licensed clinical care, clearly defined.

Skilled nursing is provided by a licensed RN or LPN and should be offered only when it is included in Carevona’s current PHCP authorization and the individual service plan.

Clinical support

Nursing procedures may include:

Medication administration

Licensed administration and monitoring of medications according to orders, applicable requirements, and the nursing plan.

Wound care

Assessment, dressing changes, documentation, and communication about wound status when ordered and included in the plan.

Catheter care

Licensed catheter-related care and monitoring within the nurse’s scope and the individual’s clinical instructions.

Health assessments

Focused nursing observations, assessments, documentation, and communication with the appropriate care partners.

Important distinction

Availability must be confirmed.

PHCP registration does not mean every agency currently provides every category. Skilled nursing depends on the agency’s authorization, licensed staff, assessed needs, and the individual service plan.
